Step 1: Fill out the Prosper Intake Form linked below with all of the requested information and send it to prosperdda@gmail.com.
Step 2: Your request for services will be reviewed by our administration team and if the company decides to move forward, a video meeting will be set to meet your child and discuss the program with you. This meeting’s goal is to find the services that best fit your child’s needs whether or not that is with our company.
Step 3: Following the video meeting, if we are a good fit for your child, the rest of the intake packet will be sent, approval and funding will be secured, and services will begin.
